All Club Forms
- Club applications are accepted in March for the upcoming year, August for the current year, and November for second semester.
- Complete this form each time that you have a club meeting. You must submit at least two minutes each month.
- All fundraisers must be completely approved 15 days before the event and the profiting organization must be made clear on all paperwork.
- Clubs must submit the fundraiser approval form and complete both pages of the form. The information recorded on the back of the form should be copied onto the second page of the packet before submitting it.
- Once the fundraiser is complete, the second page should be completed and turned in with all money raised immediately after the event.
