DRIVER FORMS ARE ONLY VALID FOR THE CURRENT SCHOOL YEAR

Each year a new form must be completed

Parent Driver Registration Form

This form is to be completed by all those parents who will be driving their athlete and other students to/from the contest. Once the form has been completed, their form will be routed to me for approval. I will send you a list of approved drivers. 

• Required attachments: current driver's license and their proof of insurance (proof of insurance MUST for the liability amount)

Student Release for Private Vehicle Transportation

This form is to be completed by all students and will be transported in a private vehicle other than their parents/guardians. 

This form is to be completed and printed for your records. I can also add you as a tracker so you can receive these forms online to save paper. You will just need to let me know.

Student Release to Transport Themselves

Both parent and student will need to complete this form. This form is ONLY for students to drive themselves to/from contests.
